K-8 ONLINE ENROLLMENT

 

                                         ONLINE ENROLLMENT FOR 2024/2025 IS TEMPORARILY DISABLED WHILE

Existing students: Parents may go into the Campus Parent Portal to update their information.

**Free and Reduced Application (Please do not fill out for the 2024/2025 School year. There will be a new one added for the 2024/2025 school year after July 1 when the state Department of Education releases the new application.)

Any additional documentation required will be requested at a later time.

 

You can scan and email the following documents to cheupel@banner.k12.ok.us  if those documents are not currently on file or if there has been a change in information:

Birth Certificate, Shot Record, Residency Affidavit

Proof of Residency (3 are required): 

  1. Proof of payment of local personal income tax or ad valorem taxes;
  2. Title to residential property in the district, or a valid unexpired lease agreement, or receipts for payment of rent on a district resident in which the applicant actually resides;
  3. Proof of provisions of utilities;
  4. A valid, unexpired motor vehicle operator’s permit or motor vehicle registration;
  5. Maintenance of voter registration;
  6. Notarized affidavit verifying residency and that the affiant has assumed the permanent care and custody of the student.  (The filing of a false affidavit shall be subject to punishment in accordance with 70 O.S. § 1-113(A)(1).)
